All forms are in a pdf format and editable for your convenience. Completed forms need to be turned into the SWSRA Office at 12521 S. Kostner Ave, Alsip, Illinois 60803 or can be faxed to (708)-389-6458. If there are any questions, don't hesitate to call SWSRA at 708-389-9423.

Please note that if the form is in black and not underlinded, it means that the form is not available at the time. A reason could be because it's out of season or not ready yet.

Master Registration Form

The Master Registration Form needs to be completed once a year and must be on file before any one is allowed to sign up for programs. The Master Registration Form must always be updated, so if there are changes please call the SWSRA office at 708-389-9423 and let us know!

Waiver and Release Form

The Waiver and Release Form needs to be completed every season when someone registers for programs. If the participant is under 18, a parent/guardian must sign.

Dispensing Medication Waiver and Dispensing Information Form

The Dispensing Medication and Dispensing Information Form is to be filled out if the participant requires the SWSRA staff to dispense medication during programs, trips and special events. This form must be completed by ALL DAY CAMP participants whether medication is dispensed or not.
